Do the following tasks to unlock the achievement and it's alloted gamerscore:

Admiral Kirk Award (50) - Complete all missions in the TOS era.
Agile Predator (20) - Complete "Behind Enemy lines" unscathed...
Allied Commander (20) - Be a team player in "Generals".
Anonymous Red-Shirt Award (5) - Be the first to go down in a 4 fleet multiplayer game.
Antique Collector (20) - Start a mission in the Next Generation era with a fleet containing a Enterprise era ship.
Autonomous Operator (20) - Complete the final mission without the use of support craft.
Avenger of Surak (25) - Destroy 10 Romulan ships in one online battle... if you can find them.
Brazen Warrior (20) - Never hide from your enemy in "Behind Enemy Lines".
Captain Archer Award (50) - Complete all missions in the Enterprise era.
Chain Reaction (20) - Deliver the killing blow to more than one ship with a single torpedo.
Custodian of Phidian (20) - Save the sensor relays in "Revelations".

Legacy Captain (20) - Unlock the USS Legacy.
Liberator of Makus (20) - Secure all enemy strongholds in "The Squeeze".

Pacifist of Makus (20) - Complete "The Squeeze" without spilling any blood...

Riker Award (40) - Complete the Next Generation era without losing a single ship.

Savior of Aurelia (20) - Protect your ward in "Anger and Mercy".
Scourge of Kathra (20) - Roll like a wave of destruction through the Kathra system...

Spock Award (35) - Complete the Original Series era without losing a single ship.
Starfleet Academy Graduate (25) - Complete the Tutorial Mission.
Starfleet Admiral (100) - Complete the single player campaign on "Admiral" difficulty.
Starfleet Captain (75) - Complete the single player campaign on "Captain" difficulty.
Starfleet Ensign (50) - Complete the single player campaign on "Ensign" difficulty.
Sword of Aurelia (20) - Take a swing at the bully in "Anger and Mercy".
T'Pol Award (30) - Complete the Enterprise era without losing a single ship.
The Wrath Of Khan (25) - Destroy 10 Federation ships in a single online battle.
Tyburn Shockwave (20) - During "Stirring the Hive", knockout as many of the enemy fleet as you can in one blow...

Unlock the U.S.S. Legacy
To unlock the secret ship "U.S.S. Legacy" (and receive the achievement); beat the game once on any difficulty then load your game. When you do; you should be on the ship buying screen right before the final mission. From there; sell and/or buy ships to make sure you have 3 Sovereign class ships. The final ship you buy will be the USS Legacy.
